In order for us to provide you with prompt service, please arrive 15 minutes prior to your appointment and bring your insurance card, social security number, and a filled in Patient Information form with you. We’ll also need to know if you have any medical conditions, such as artificial joint replacement or heart valve replacement, that require antibiotic premedication. If you are taking any blood thinner medications, please consult with your doctor before your appointment.
